I use this myself and it's working okay, all fields are showing so I am not sure what is going on with yours.

I don't know what is the problem, but I find one uploaded here, little bit older (v2.1.1) version, the old working. After installing v2.1.2, the pro options not showing up. Now i using the oldest version.

Issue with scrolling to the top within submitting the form;
Issue with a signature field on multi page forms;
Issue with displaying signature field in the lightbox;
Small CCS issues in the admin panel.

After installing this plugin, it asks for activation. I installed version 2.1.3.1 and tried it. I am sad.
