You can to release this task with native contact form. Add button to product page that redirect to contact form. In Contact form add menu "Found lower price". And you will have all features from module in question.
Also you can use module "Form builder" but I am not sure in result with all your conditions

Never buy modules from prestadev.pl they are very bad, no optimized for speed + 0 support after buy, and they wont to back cash
